SUMMARY:Sickle Cell Transition Camp for High School Students in Chicago
DESCRIPTION:Free two-day sickle cell transition camp to help prepare youth 
 who will be moving from pediatric care to adult health care\n\nThe Sickle 
 Cell Disease Association of Illinois (SCDAI) and the University of Illinoi
 s Hospital (UI Health) will host "Sickle Cell Transition Camp" on Aug. 4-5
 . The camp will run from 10 a.m. to 3 p.m. daily.\n\nThe two-day camp is f
 or high school students. Teens in their freshman through senior year will 
 gain a better understanding of "transition" while having fun together. Tra
 nsition means that individuals stop receiving healthcare services from ped
 iatric professionals and move to adult healthcare providers.\n\nActivities
  will include:\n\n 	Meeting other teens with sickle cell disease\n 	Partic
 ipating in a wide range of activities\n 	Exploring transition topics and t
 he move from pediatric care to adult care\n 	Eating lunch together\n\nThe 
 camp is free to attend.\n\nAll camp participants must be dropped off and p
 icked up at SCDAI's office daily.\n\nSCDAI will host a virtual information
  meeting for campers and their families closer to the start of camp.\n\nRe
